Dual Special Needs plans (D-SNP)

D-SNPs are for people who have both Medicare and Medicaid. A dual plan works together with your Medicaid plan. You keep all your Medicaid benefits. Plus, you could get many extra benefits and features for as little as $0 per month. Dual Special Needs plans have a $0 premium for members with Extra Help (Low Income Subsidy).

What is a Dual Special Needs plan (D-SNP)?

A Dual Special Needs plan (D-SNP) is a type of health insurance plan. It’s for people who have both Medicare and Medicaid. These plans are designed for people who may need more help because of disabilities, age or health conditions.

With a D-SNP, you could get more benefits than with either Original Medicare or Medicaid alone. You’ll keep all your Medicaid benefits, plus add even more at no extra cost to you.

Extra benefits may include:

  • Routine dental care1
  • A monthly credit to spend on healthy food, over-the-counter products and pay utility bills2
  • Routine eye exams, plus credit for eyewear
  • Routine hearing exams, plus credit for hearing devices3
  • Rides to health care visits and the pharmacy4

Am I eligible for D-SNP?

You may be eligible to qualify for a dual plan if:

  • You enrolled in Medicaid in your state
  • You have Medicare Parts A and B (Original Medicare)
  • You live in the dual-eligible plan’s coverage area

How do I enroll in D-SNP?

To enroll in a dual-eligible plan online, you can fill out and submit an online enrollment application. The person who is applying will be asked a series of questions and the system records the answers.

Benefits, features and/or devices may vary by plan/area. Limitations, exclusions and/or network restrictions may apply. OTC, food and utility benefits have expiration timeframes. Review your Evidence of Coverage (EOC) for more information. The healthy food and utilities benefit is a special supplemental benefit only available to chronically ill enrollees with a qualifying condition, such as diabetes, cardiovascular disorders, chronic heart failure, chronic high blood pressure and/or chronic high cholesterol, and who also meet all applicable plan coverage criteria. There may be other qualified chronic conditions not listed. Review your plan Drug List (Formulary) on UHC.com/Medicare for a list of covered prescription drugs, including those on Tier 1. Dual Special Needs plans have a $0 premium for members with Extra Help from Medicare (Low Income Subsidy). Some of the benefits on a Dual Special Needs plan may also be available from the state Medicaid program or a managed Medicaid plan.
