A team approach to health care
As a UnitedHealthcare Medicare Advantage member, your plan may include a team approach to your health care, with both your provider and UnitedHealthcare working together. This approach is designed to provide you with a dedicated care team to support your unique health needs, from preventive care to mental health care.
With this team approach, your provider may have additional support from one of many health care organizations,3 including the ones listed below, to help manage your overall well-being.

At UnitedHealthcare, we understand managing your overall well-being includes support from your provider and their care team. By working with certain health care organizations, we can help members get the care and support they may need to make the most of their health care experience.
No matter how complex the care, health care organizations can help simplify the path to better health. The care team will:
- Guide members through their health journey
- Help connect members to a specialist, if needed
- Help with the transition back home if members go to the hospital or a long-term care center
- Provide care coordinators and case managers to help with referrals, appointments and reminders
- Provide members with local community resources and support
When a member enrolls in a plan or chooses a primary care provider (PCP) in the UnitedHealthcare network, they are part of a robust, coordinated care system. Coordinated care ensures the member’s PCP and specialists are working together with health as the focus.
UnitedHealthcare and health care organizations use a unique approach to coordinated health care to better serve Medicare Advantage patients and help to deliver better health outcomes, including:4
- Lower chance of emergency room (ER) visits
- Lower chance of in-patient admission
- Lower chance of 30-day hospital readmission
Members become part of a health care organization network after choosing a Medicare Advantage plan that includes the health care organization network or selecting a PCP within a health care organization network.
Members may receive communications directly from the health care organization or their provider welcoming them to the network. Additionally, members might receive clinical reminders, local event invitations and claims or prior authorizations directly from their provider.
Members can use the UnitedHealthcare app or sign in to their member site to find answers to common plan and benefit questions. Members can also contact UnitedHealthcare Customer Service by calling the number on their UnitedHealthcare UCard®. Members may want to call their provider for any clinical or appointment scheduling questions.
