UnitedHealthcare Choice/Choice Plus
For groups with any number of employees
These open access plans give employees and their covered family members the freedom to see any doctor or specialist in our large national network without a referral. Employees also have convenient access to virtual primary care — it's like an office visit with a primary care provider (PCP) — only over a computer or mobile device instead.
- With Choice plans, employees must receive care from network providers for benefits to be covered; out-of-network coverage is for emergency services only
- Choice Plus plans include coverage for out-of-network providers at a lower benefit level

UnitedHealthcare Core/Core Essential
For groups with any number of employees
Open access plans supported by a national network configured to meet local market dynamics. They offer employees and covered family members their choice of providers and facilities within a defined network, with optional out-of-network coverage and tiered benefits available. Employees also have convenient access to virtual primary care — it's like an office visit with a primary care provider (PCP) — only over a computer or mobile device instead.
UnitedHealthcare Navigate® and UnitedHealthcare Charter®
For groups with any number of employees. Not available in all markets or states or for all funding types. Contact your broker, consultant or UnitedHealthcare representative for availability. Navigate and Charter include a variety of plan designs that provide traditional benefits with the guidance of a primary care provider (PCP).
- Both have their own national network, which may be more focused compared to the Choice and Core networks
- In some markets, Charter plans are available in collaboration with local health systems to help streamline access to quality care

NexusACO®
For groups with any number of employees. Not available in all markets or states or for all funding types. Contact your broker, consultant or UnitedHealthcare representative for availability.
NexusACO plans offer access to quality providers and, in some markets, collaborate with local health systems to help streamline access to quality care. Multiple plan designs are available, including open access plans with no referrals required, as well as PCP-guided plans with referral requirements.
UnitedHealthcare Doctors Plans
For groups with 100+ employees in Arizona, Northern California, Colorado and Washington only. Not available for all funding types.
In collaboration with local health systems, these plans are built around the doctor-patient relationship. PCPs guide employees to the care they need and help them avoid cost surprises. Benefits may include same-day appointments (virtual or in-person) and $0 copays for services employees use more often, like PCP visits and urgent care.
UnitedHealthcare Benefit Ally®
For groups with 100–5,000 employees. Not available in all markets or states or for all funding types. Contact your broker, consultant or UnitedHealthcare representative for availability.
Benefit Ally provides medical coverage plus supplemental benefits bundled to simplify the administrative process for you. A preset suite of products (Critical Illness, Accident and Hospital Indemnity) that must be bundled with an eligible base medical plan, Benefit Ally automatically pays out a financial benefit to employees and covered family members for qualified medical events.
UnitedHealthcare FlexWork®
Built for restaurant, retail, hospitality and staffing businesses with variable-hour, part-time, limited-time or seasonal workers, UnitedHealthcare FlexWork is designed to make healthier possible for everyone at your business. Customize your benefits package from a suite of medical and specialty plans for your non-benefits-eligible and benefits-eligible W-2 employees.
UnitedHealthcare Preventive Plan
For groups with 100+ employees and administrative services only (ASO) funding arrangements.
A limited benefit plan with coverage focusing on 100% preventive care and medications, the Preventive Plan meets the minimum standard requirements for value and affordable health care coverage that certain employers are required to offer.
The Surest health plan is an ACA-compliant, copay-only health plan solution available to employers with 51+ employees and designed to give employers the opportunity to offer a sustainable, intuitive health benefit without shifting costs to employees. Surest leverages the nationwide UnitedHealthcare provider network. Available self-funded and fully insured; available in all markets (except for Hawaii and Puerto Rico) for self-funded.

Getting more value with tiered benefits
Tiered benefits are available with some UnitedHealthcare plans to help encourage employees and covered family members to receive their care from certain providers and facilities that may offer the greatest value, with the flexibility to access a larger network at a higher cost.
- Premium tiering encourages employees to choose UnitedHealth Premium® physicians evaluated for quality and cost efficiency
- Hospital tiering encourages employees to choose Premium designated providers, specialists and hospitals (market restrictions apply)
- Place of Service differentials deliver savings when employees get health care services from a freestanding facility instead of a hospital-based facility
- Preferred Lab tiering allows employees to get lab services at no additional cost when services are delivered by a Preferred Lab facility
Health plans with surplus refund potential
Designed for more flexibility and simplicity, UnitedHealthcare Level Funded and All Savers® Alternate Funding plans are built to give businesses more ways to save. Because these plans are based on your plan participants’ actual medical claims, your health plan may get a surplus refund at year-end if those claims are lower than expected.3
